In order to complete this install please fill out the details requested below.
Please note the following points before you continue:
    1. Change the permission (chmod) of 'includes/config.php' to 777. (After installation dont forget to change it back to 644)
    2. Change the permission (chmod) of the 'imagehosting.css' to 777.
    3. Change the permission (chmod) of the folder 'images' to 777.
    4. Change the permission (chmod) of the folder 'gallery' to 777.
    5. Change the permission (chmod) of the folder 'tmpeditimages' to 777.
    6. Change the permission (chmod) of the folder 'zip' to 777.
    7. The database you install into should already exist.
    8. Provide a database prefix to distinguish the IMAGE GALLERY tables from other tables or if you have some similar tables.
    9. After the install process delete the 'install' folder and its contents.
